Broward Health ACO Services

Broward Health ACO Services, Inc. is an accountable care organization which serves to better manage the health of populations, achieve efficiencies in care delivery and improve overall experience and satisfaction of patients and their families. As an ACO Broward Health has entered into a commercial contract with Cigna as of January 1, 2015, Florida Blue as of December 1, 2015 and Broward Regional Health Planning Council (BRHPC) as of January 1, 2017. Our goal is to improve patient access to health care, enhance care coordination, and achieve the goal of improved health, affordability enhance patient experience and offer providers continued medical education activities. Patients benefit from improved care coordination and greater emphasis on preventive care.

Our collaboration with Cigna and Florida Blue helps close gaps in care such as missed health screenings or prescription refills, cultivating the appropriate use of hospital emergency rooms, increasing the number of preventive health visits and improving follow-up care for people transitioning from the hospital to home.

Broward Health ACO Services, is collaborating with South Florida physicians that aim to improve the quality of patient care and satisfaction. To accomplish this, the participating health plans will advise ACO physicians and Broward Health should potential gaps in care be identified of your aligned patients. These aligned patients, are closely monitored by the health plans for compliance with preventive and follow up care. Broward Health ACO Services will communicate with you and your office staff providing updates, reports, and assist in gathering patient level information when applicable, such as completion of evidenced based preventive screenings.

Our population health team consists of RN care coordinators, and mental health professionals who will collaborate with you to assess care needs and create a patient care plan as appropriate. We will work to identify any potential barriers a patient may have and assist in assuring appropriate resources, (i.e.: Home Care, Social Services, Pharmacy, and Behavioral Health) are provided. When your patients are hospitalized, we will work with the discharge planners at the hospital to ensure that you have the transition of care information needed to maintain continuity of care.

What is an Accountable Care Organization?
An Accountable Care Organization (ACO) is a group of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their patients. An ACO’s goal is to improve quality and efficiency of care provided and demonstrate increased value from healthcare expenditures.
What are the benefits for an ACO Provider?
Broward Health will support you through the identification of beneficiaries, provision and analysis of a beneficiary’s claims data, as well as providing you with evidence based guidelines and care management workflows to address the most prevalent diagnoses and/or highest risk patients among the population (e.g., heart disease, hypertension).
What is an aligned patient?
A patient that has elected to utilize an ACO participating physician for their primary care services.
How do my patients benefit?
Benefits do not change and there is no additional charge to your patient. Your patients will receive enhanced coordinated and collaborative care. Who is leading the ACO?
The ACO is overseen by a Board of Directors. The ACO will also establish committees to oversee and provide input on quality, care coordination, evidence based guidelines and patient engagement. Community physicians may be invited to participate on these committees.
Am I at risk if the ACO experiences losses?
No. There is no downside or financial risk associated with participation and no financial investment is required to join the ACO.
How will this impact my office operations?
We will provide you with a list of patients that have been aligned to your practice, and provide ongoing updates regarding the key performance indicators established by the ACO. Our care coordinators may also contact your patients to provide guidance as they navigate through a hospital discharge plan and outpatient care management. Our goal is to work as an extension of your practice and help bridge any gaps in care.
With which commercial payers are you collaborating?
We have a collaborative agreement with Cigna and Florida Blue. We are also looking to collaborate with other payers in the near future.
